UWSP Customized Cellular Agreement

This form allows you to sign up for UWSP Customized Cellular Service.  The UWSP Telephone Support Office reserves the right to deny customized service.  Only a limited number of Customized Contracts available.  Service will be "first come, first serve."  Once this form is submitted, the Telephone Support Office will contact you within one (1) business day.  If you do not receive a response, please call the Telephone Support Office at (715) 346-2562.

If you want a contract for more than 9 months, you need to fill out a regular UWSP Cellular Agreement.

Note:  The cellular month runs from the 27th through the 26th.  If you sign up for monthly service, your contract will always conclude on the 26th, at midnight.  If you sign up for weekly service, you must indicate a Contract Start Date in the next section.  If you wish to extend your contract past the initial terms, you will need to contact the Telephone Support Office prior to cancellation.

On what date would you like to begin your customized contract?  If you do not enter a date, the Telephone Support Office will assume the contract will begin on the date this contract is submitted.  If you select one of the weekly options, your contract will end 7 days, 14 days, or 21 days after the date indicated.

Cellular Phone Number
 

With a customized contract, you will be assigned a Stevens Point cellular number.  With a customized contract, the Telephone Support Office cannot port (transfer) numbers, nor can we assign a number local to another area.  If you require a ported number, or a number local to another area, then you must fill out a regular UWSP Cellular Agreement

Phone Selection/Wireless Insurance

Phone selection is limited.  Phones are available first come, first serve.  You will be required to return all equipment (phone, chargers, manual, etc.) at the end of the contract. 

Wireless insurance is required on all phones.  The charge is $5 per month.  Wireless insurance will protect your phone from loss, damage or theft.  If your cell phone is lost, damaged, or destroyed, you will be required to pay the insurance deductible of $50.

If you sign up for a weekly contract, the features listed below are not available.  Your contract is enabled with pay-per-use text, picture, and video messaging.  Pay-per-use text messaging is $.15 per message.  Pay-per-use picture, or video, messaging is $.25 per message.

Acknowledgement

I understand and accept the terms and conditions of this agreement.  Telephone Equipment:  I will be required to return all equipment (phone, chargers, manual, etc.) at the end of the contract.  Should I damage or destroy, the phone, I will pay the insurance deductible.  Cellular Plan:  I have read the plan descriptions and I understand the charges.  To read a complete list of Terms & Conditions, click here.

If this order is placed by 12:00 p.m. (Noon), during normal business hours, the Telephone Support Office will do their best to fill this order before 4:00 p.m.  There are no guarantees.  You will be notified by telephone, or e-mail, when you can pick up your phone.
