UWSP Cellular Renewal Form

This form allows you to renew your UWSP Cellular Agreement.  This is a 9-month contract period, with month-to-month following the initial period.  To cancel your service you must fill out the Cellular Service Cancellation Form.  To change plans you must fill out a Cellular Plan Change form.  To add or remove features you must fill out a Cellular Feature Change form.

Once your form is submitted, the Telephone Support Office will contact you within 1-2 business days.  If you do not receive a response, please call the Telephone Support Office at (715) 346-2562.

One-year manufacturer's warranty included.  With a contract renewal you are only allowed one phone within a 16-month period.  If you cancel your service before the 16-month period, you will need to return your phone.  You will get reimbursed for a portion of the phone cost.  Reimbursement is based on condition and length of service.

Acknowledgement

I understand and accept the terms and conditions of this renewal agreement.  Early Cancellation Fee:  If I cancel my service prior to the expiration of my term of nine (9) months, I may be charged an early cancellation fee of $200.  Telephone Equipment:  I understand that I am only allowed one telephone purchase (at the contract rate) within a 16-month period.  I must return my phone if I cancel before the 16-month term.  Cellular Plan:  I have read the plan descriptions, and I understand the charges.  To read a complete list of Terms & Conditions, click here.

Once this order is placed, your cellular phone will arrive in our office within 2-3 business days.  You will be notified by telephone, or e-mail, when you can pick up your phone.
