UWSP Cellular Agreement

This form allows you to sign up for UWSP Cellular Service.  This is a 9-month contract period, with month-to-month following the initial period.  Your service does not automatically stop.  In order to cancel service you must fill out a cancellation form.  If you do not need a contract for 9 months, please review our Customized Cellular Service.  Once your form is submitted, the Telephone Support Office will contact you within 1-2 business days.  If you do not receive a response, please call the Telephone Support Office at (715) 346-2562.

ALLTEL Customer or Port-In Request

Are you currently an ALLTEL customer and wish to keep your number?  Or, do you want to keep your number from another provider?  If so, you will not be able to complete this on-line form.  Stop by the Telephone Support Office to complete the necessary paperwork. 

*Note:  Alltel customers do not have to pay an activation fee.  If you have a phone from another provider, be sure of your cancellation policy with that provider.

Cellular Phone Number

The Telephone Support Office will assign a cellular number, or numbers, local to Stevens Point.  You will be unable to request a number.  If you want a cellular number local to another area, please enter the Wisconsin city.

*Note:  Some Wisconsin areas will not be available and numbers outside of Wisconsin are not available.  If your city in unavailable a Stevens Point number will be assigned.

Shared, or add-a-lines, are only available with certain plans.  Please review each plan carefully.

All plans are National Freedom (nation-network), offering extensive national coverage.  All plans offer unlimited nights and weekends and mobile-to-mobile (Alltel customers only).  This is a 9-month contract period, with month-to-month following the initial period.  Your contract does not automatically stop at the end of the 9-month period.  To cancel service you must fill out a cancellation form.  By selecting one of the plans, you are aware of all charges associated with each plan.

700 Minutes ($43.95/mo) - add one additional line at $15.00/month

*Note:  If you exceed the allotted minutes you will be charged $.45 per minute in overages.

Activation Fee & Cancellation Policy

 

The activation fee for each line is a one-time charge of $25.  This amount is non-refundable.  If you are a current ALLTEL customer, this fee is waived.  The UWSP cancellation fee is $200.  A 15-day/30-minute cancellation period has been established (whichever occurs first).  Within this period, you can cancel your plan without incurring a fee.

If you agree to pay the $25 activation fee(s) and you understand the cancellation policy, please enter your initials.
 


If you do not agree to the $25 activation fee, and you do not understand the cancellation policy, do not submit your contract.  Please contact Mary Glinski at (715) 346-4819 with questions.
 

Acknowledgement

I understand and accept the terms and conditions of this agreement.  Early Cancellation Fee:  If I cancel my service prior to the expiration of my term of nine (9) months, I may be charged an early cancellation fee of $200.  Telephone Equipment:  I understand that I am only allowed one telephone purchase (at the contract rate) within a 20-month period.  I must return my phone if I cancel before the 20-month term.  Cellular Plan:  I have read the plan descriptions, and I understand the charges.  To read a complete list of Terms & Conditions, click here.

Once this order is placed, your cellular phone will arrive in our office within 2-3 business days.  You will be notified by telephone, or e-mail, when you can pick up your phone.

By entering your initials you are accepting the terms and conditions of the UWSP Telephone Support Office.  This is your electronic signature, and acceptance.
