This form allows you to apply for UWSP Business Cellular Service.  Once your form is submitted, Mary Glinski will contact you within 1-2 business days.  If you do not receive a response, please call Mary at (715) 346-4819.

New phones arrive within a week.  Special orders may take longer.  The phone will be shipped to the Telephone Support Office.  You will be notified when it arrives, and when you can pick it up.
