This form allows you to switch from one UWSP long distance plan to another. All long distance plans are effective the 1st of the month. No plans can be changed after the 15th of each month. If filling this form out after the 15th, the change in plan will become effective the 1st of the next month. If you have any questions, please call (715) 346-2562.
