UWSP Cellular Service Cancellation Form

This form allows you to cancel your UWSP Cellular Service.  When you completed your Cellular Service Agreement, you agreed to a 9-month contract.  After 9 months, your contact continues month-to-month.  If you cancel within the initial 9 months, you are subject to a $200 cancellation fee.  After the 9-month period, you can cancel at any time without incurring a cancellation fee.  However, this form must be completed to cancel your cellular service.  The Cellular Cancellation Form must be received by 3:00 p.m. on the 26th, to take effect the following month.  If the 26th falls on a weekend, or holiday, the first business day before the 26th.  If you have any questions, please call (715) 346-2562.
