You must complete this form to cancel, suspend, or transfer cellular service with UWSP. A form must be completed for each number on your contract. Please refer to your Cellular Service Agreement and the Terms & Conditions.

For each cellular number, the customer agreed to a 9-month term. The UWSP Telephone Support Office agreed to a 20-month term with the provider. For a renewal, the customer agreed to another 9-month term and UWSP agreed to another 16-month term.

If you voluntarily cancel service prior to the 9-month term, you will incur a $200 cancellation penalty, per number. By paying this penalty, the number is completely cancelled with UWSP and may be transferred to another provider. The device and chargers do not need to be returned.
If you cancel after the 9-month term, but before the UWSP term expires, you will not incur the $200 cancellation penalty, but you must return the device and chargers. The device must be returned in good, working condition. If the device is deemed vandalized or damaged, you may be charged the replacement price.
If you cancel after the 9-month term, and after the UWSP term, you are not required to return the device. You are allowed to transfer your number to another provider.
Temporary Suspension - If you would like to temporarily stop billing on your cellular service, you must have prior approval from the UWSP Telephone Support Office. Temporary suspension must be a minimum of three months. Approval is based on length of contract (9-month term complete), payment history, and reason for suspension (i.e. semester abroad, military service, etc.). Stop by the Telephone Support Office to discuss your options for a temporary suspension.

Cancellation: The cellular month runs from the 27th to the 26th. All* billing goes through the 26th of the month.	Cancel cellular service immediately. I will pay the $200 cancellation fee. Cancel cellular service at the earliest date without incurring the $200 fee. Transfer cellular service. *You must meet the terms of your agreement.
Reason For Cancellation:	Graduating - Moving outside of Stevens Point. *Note: UWSP Graduates may retain UWSP Cellular Service. Switching Provider - better deal with a different provider. Dissatisfied with UWSP service. Other - Please list a brief reason.
