UWSP Cellular Plan Change Form

This form allows you to switch from one UWSP cellular plan to another.  All plans begin on the 27th day of each month.  Plan changes must be received by 3:00 p.m. on the 26th, to take effect the following month.  If the 26th falls on a weekend, or holiday, the first business day before the 26th.  If you have any questions, please call (715) 346-2562.
