Please use this form if you would like to receive a telephone authorization code.  An authorization code will allow you to place telephone calls from any telephone on-campus.  The 7-digit code will be billed directly to the account information provided.  Once this form is submitted, your authorization code will be sent to the e-mail address provided, within one business day.  Remember . . . you will be responsible for all charges associated with the Authorization Code.  Treat the code like a credit card.  DO NOT share it with anyone.
